The latest driver is 52.43 from MOS 3.19. I don't think that your gra-card is the reason.
For what I can see, your partitioning is OK. It's crazy that it does not boot! I can only imagine that you did something wrong in ENV-variables and/or NVRAM.
Please post here folowing again: Environmentvariables, NVRAM and the device-alias:
> printenv
> devalias
Your HD is mounted to the 1st SATA-Port? If not, please do and for test put all other drives away (if there any).
-----------------------
If we don't find the reason, don't give up. There is a second way: we can do it with a forth-bootmenue. This should work in any case.
Here a output from my devalias! The block beginning with sata is interessting for you.The rest is not really necessary for booting.

And here my ENVVAR:
I use a boot-menue, so boot-device and boot-command should be hd:,\:tbxi and mac-boot in your case.

Normaly on G5-OF there is no need for any NVRAM-definitions!
Peg2, 3xPowerMac G5, 2xPowerbookG4, 2x MacMiniG4, Efika (again), A3000T and life is never boring.....
